The exporter must fill out and sign a NAFTA Certificate of Origin before claiming a reduced duty preference under the North America Free Tarde Agreement for an imported good.

• Complete and sign the NAFTA certificate of origin on page 1 by following the instructions on page 2.

We are required by customs regulations to obtain a valid power of attorney from you before operating any customs business on your behalf.

• Print and read all pages of the document carefully.
• Complete the power of attorney on page 1 and the importers profile on page 2
• Sign and date the bottom on page 1.
